3 1 Education Model Announcement CURRENT

Vocational Education Announcement within the Scope of 3+1 Education Model (Updated)

Dear Students,

Our students enrolled in the "Workplace Education" and "Workplace Application" courses within the scope of the applied 3+1 education model in the Spring semester of the 2023-2024 Academic Year;

IN ORDER TO APPLY TO THE BUSINESSES WHERE THEY WILL APPLY, THE FOLLOWING DOCUMENTS WILL BE OBTAINED AND APPROVED BY THE BUSINESS.

IF THE FEE WILL BE PAID; and if the workplace wants to benefit from state support; After completing and signing the documents numbered 3 and 4 specified below, they will submit them together with the documents numbered 1 and 2.

1- Vocational Training/Internship/Practical Course Application Form in Business Administration (with 3 wet signatures)

2- Eligibility certificate to be obtained from e-government (To be delivered only to student affairs)

3- Agreement on Applied Training Principles and Obligations of the Parties (3 wet signed)

4- Vocational Training/Internship Wage Incentive Application form in Business (with 3 wet signatures)

IF THE FEE WILL NOT BE PAID:

In addition to documents 1, 2, 3 and 4

5-Business Fee Waiver Form

This document will be prepared in 2 copies. If the student will not be paid a fee by the business; The student must fill out the waiver form stating that he/she will not charge any fee to the business and submit one copy to the business. Another copy must be submitted to our School along with document number 6.

6-Student Fee Waiver Form

After the student completes and signs this document; It must be submitted to our School along with document number 5.

It must be delivered to our School by hand, post/courier or via our school's e-mail address (provided that the originals of the documents are sent) by the end of working hours on Friday, February 9, 2024. The necessary forms are attached to the announcement.

After the Internship Application documents are completely filled out and signed by the business/institution and the student where the internship will take place, it will be in your interest to send them to the school's e-mail address (susurlukmyo@bandirma.edu.tr) and have them checked, and to have the documents sent to you again after the Higher School Approval, in order to speed up your process.

As a result of the check, there are no errors, missing documents, missing information, etc. in your documents. If there are any, you will be notified and you will be asked to re-send after completing the missing parts. After the College Approval is completed, documents will be sent to you via e-mail and you will be given to the business.

Vocational Training/Internship/Practice in Business Administration

Start date: 26 February 2024

End date: June 16, 2024

IMPORTANT NOTE: In order for payments to be made, the originals of all documents (with wet signature) must be submitted.

Documents that must be submitted during the application before the internship:

1-Vocational Training/Internship/Practice Course Application Form in Business Administration (3 copies, with wet signature)

After all information is filled in completely and legibly and signed by the business, 1 copy will be delivered to the business, 1 copy will be delivered to our School, and 1 copy will remain with the student.

2- Certificate of eligibility to be obtained from e-government

(If this document is missing, the student's application will not be accepted.)

3-Contract Provision

It will be delivered to our School after it is signed by the student and the business official.

4-Internship Fee Incentive Application Form

It must be filled out completely by the business and YES or NO must be specified in the Business's Request to Participate in State Contribution Application section.

5-Business Fee Waiver Form

This document will be prepared in 2 copies. If the student will not be paid a fee by the business; The student must fill out the waiver form stating that he/she will not charge any fee to the business and submit one copy to the business. Another copy must be submitted to our School along with document number 6.

6-Student Fee Waiver Form

After the student completes and signs this document; It must be submitted to our School along with document number 5.

Documents to be submitted at the end of the internship

1-Unemployment Fund Contribution Student and Employer Information Form

After the Student and Business information is filled in completely and signed by the Workplace Official, it will be delivered to our School together with the receipts. (The receipts must be attached. If the receipts are not delivered, State Contribution payments will not be made.)

2-Business Evaluation Form

This document, which is intended for the evaluation of the student, must be filled in the Student information section and delivered to our school by the Business Administration in a CONFIDENTIAL and sealed envelope.

During the submission of the Internship Notebook, the student can also submit this document in a sealed envelope.

3-Student Evaluation Form

Relevant fields must be filled in by the student and submitted to our School.

4-Internship File

1- If the workplace will pay a monthly fee to the student based on the amount specified in the contract and the workplace will request a part of this fee from the unemployment fund within the scope of state support, follow the steps below.
